Lateral epicondylitis is a condition characterized by inflammation or degeneration of the tendons that connect the forearm muscles to the outside of the elbow, specifically the wrist and finger extensor tendons. The most commonly affected tendon is that of the extensor carpi radialis brevis muscle. This problem typically arises from repetitive movements or overuse, not only in racquet sports but also in occupational activities (tool use, keyboards) or household chores.
The most common symptoms include:
- Pain on the outside of the elbow that can radiate down the forearm and wrist.
- Tenderness to the touch on the lateral epicondyle (the prominent bone on the outside of the elbow).
- Pain when lifting objects, making a fist, rotating the forearm, or extending the wrist against resistance.
- Weakness in grip strength.
The Physiotherapy Approach: Beyond Pain

- Extend the affected arm forward with your palm facing down.
- With your other hand, grasp the fingers of the extended hand and gently pull them down, flexing your wrist.
- You should feel a gentle stretch in the top of your forearm. Hold for 30 seconds, repeat 3 times.

Isometric Wrist Extension Exercises:
- Rest the affected forearm on a table with your hand hanging off the edge, palm down.
- Place your other hand on the back of your affected hand, as if you were going to push down.
- Try to lift your wrist upwards against the resistance of your other hand, without moving the wrist. Hold the contraction for 5-10 seconds, repeat 10 times.

Eccentric Wrist Extension Exercises:
- With a very light dumbbell (0.5-1 kg) or a can, rest your forearm on a table, palm down and wrist hanging.
- With your other hand, lift the weight to the highest wrist extension position.
- Now, slowly lower the weight using only the muscles of the affected forearm (this is the eccentric phase). Control the movement for 3-5 seconds.
- Use your unaffected hand to lift the weight back up and repeat the slow descent. Perform 3 sets of 10-15 repetitions.

Grip Strengthening:
- Gently squeeze a stress ball or a rolled towel for 5 seconds and release.
- Perform 3 sets of 10-15 repetitions.
